Population
Kawartha Lakes OHT will focus initially on enhancing coordinated care and services for seniors 65 years and older who:
- Live alone or have a poor support network
- Are frail, with complex medical needs or are cognitively unwell
- Have been hospitalized within the last year
As the work of our team evolves, the population we serve will expand and we will continue to add additional services to our site.
Our mission is to coordinate and connect care to create a seamless experience for patients and families in the City of Kawartha Lakes region.
