Wellness Connections Program

The Wellness Connections Program is designed for people aged 65+ to learn about healthy changes and choices to age well and to connect to local service providers offering supportive programs. The program can be accessed virtually through our website or with in-person support.

There is no charge for this program and new clients are always welcome! If you would like to refer yourself or someone else for the Wellness Connections Program, complete the referral form below or call our Wellness Connections Coordinator at

Online Learning Platform

When you join our Wellness Connections Program, you are given access to our Online Learning Platform. Here you can learn about topics that can help you prevent frailty and promote aging well. You can find out about local programs and supports, and also develop a wellness plan to make healthy changes and choices.

AVOID Frailty Framework

The Wellness Connections Program is created based on the principles of the AVOID Frailty framework, which is designed to improve care and quality of life for older residents as well as to support families and caregivers.
