Virtual Wellness Connection

Based on the AVOID Frailty Framework, our Virtual Wellness Connection program helps connect older adults to in-person and virtual health supports to prevent frailty and age well at home. This program assists and educates people with technology and internet connectivity skills to improve health and wellbeing.

There is no charge for this program and new clients are always welcome! If you would like to refer yourself or someone else for the Virtual Wellness Connection, complete the referral form below or call our Virtual Wellness Connection Coordinator at
